I get the feeling that it was originally written in Japanese, because some of the questions are... a little off based on the traditional American experience.
Who did you first cosplay as?
Technically? I dressed up as Mendoza from "Cities of Gold" for Halloween when I was like... 7. And that's even an Anime, techinically (太陽の子エステバン). I also did Jadzia Dax from DS9 and Susan Ivanova from B5 all before the age of 15. My first anime cosplay at an official convention (that wasn't just tossed together in a hotel room at-con) was Arisugawa Juri.
When and where did you first cosplay?
Halloween as a kid. Juri was at NDK... 4. I think.
When and where was the last time you cosplayed?
September 13-15, 2008. Nan Desu Kan, Denver. (Though that will change after next week.)

Who is your favourite person to cosplay as?
Mmm... that's really REALLY hard. But... I think I have to pick Yuushi. He's recognizable. He's comfortable. There are things that I love about wearing a LOT of my characters, and Akram runs a very strong lead on many of the others.
Who do you REALLY wanna cosplay as?
Well, at the moment, I'm really looking forward to wearing Masaomi.
Other than that, there are lots... but Rikkai jerseys are always on my to-make list. Right now I'd really like to lose some weight so as to look better in the costumes that I have. And if I was ever confident enough of myself and had the right materials and a group, I'd love to do a set of Arashi costumes. LOVE. ^^

Have you ever walked around in public in your cosplay outfit?
...both appropriately and innapropriately. Yes. Though I tend not to go shopping in costume and things like that, except when necessary at conventions for emergency supplies. There's always the necessary trips to IHOP/Perkins/TGIFridays/Wendy's in costume though, that's just how it is. I also Cosplay at work in October.
Have people in public ever recognized you and asked for photos?
In public? Never. Well... unless you count people recognizing me at work (only Yuushi)... and they didn't want photos since I was running a register. :3
Have you ever made your own cosplay outfit?
Uh..... yes. All of them since Juri. Along with the major sewing parts of at least 10 more. For various friends and group members.
What was your favourite cosplay experience?
I think standing in the elevator having a slightly inebriated Kiuchi Hidenobu staring at me and going 'Oshitari' over and over again slightly beats out meeting Higuri Yuu and hearing Jess get called 'suteki'. Just barely. ^^ The awards at NDK were pretty awesome too.
What is your worst cosplay experience?
.....I don't deal with group-stress well when I don't feel supported. That's all I'm saying. Overall, I don't have any costumes that I don't like and enjoy though.
When and where did you first learn of cosplay?
....as 'cosplay'? Probably going to NDK 2 way back when.
What did you think of it then?
"Next year, I'm making a costume to wear too!"
What do you think of it now?
Well, when the girls at work ask me to explain what on earth I'm buying I explain the costume, tell them that they won't recognize it and then say "Yeah, in my spare time, I'm secretly a freak. With an OCD obsession with detail."
Where do you usually cosplay?
Conventions. Though I like smaller photoshoots and if I were in Japan, I'd definitely hit more small events and 'meets'.
Have you ever done a cosplay skit on stage?
Yes. And it was awesome. I'm generally more of a walk-on girl though.
Have you ever won a prize for said skit?
No. But I've won craftsmanship prizes for walk-ons.
Do you plan your cosplay outfits waaay before you’re meant to wear them?
Well... on the way home from NDK last year (September 15th?) we went to IHOP and stated planning Masaomi and Tomomori. Both versions. Izayoiki Masaomi got done this week. The full armor versions are planned for 2010. So... yes.
Have you ever been to a convention and cosplayed?
Um... a bit late to be asking that after the above questions, isn't it? Yes
Ever have something terrible happen to your outfit?
Mmm. Seigaku letters. Messing up Shinomiya's wig. Akram's applique starting to fray. Not much.
What is the best thing about cosplay?
Honestly, getting to wrap the clothes of a beloved character around you and hide inside them for a while. I love it.
What is the worst thing about cosplay?
Idiots. Realizing how much of a freak I am, even among the freaks. ("Oh wow... where did you get that fabric?" "Um... I don't know, my friend bought the costume on ebay.")
Why do you cosplay?
For Love. Of the character, of the experience, of meeting that one girl who goes OMG Haruka (or whatever). Just love.

Printed up the T-shirts for Detour Karaoke today (I <3 my Yudu). I kinds wish that they said 'Detour Karaoke Team' instead of Anime Detour Karaoke, but... mah. they're done and they look awesome. Just have to heat-set the ink so that it'll last. :D
